Type 1 Diabetes Clinic
Insulin pumps, continuous glucose monitoring, and education, coordinated by a dedicated team here in the Annapolis Valley.
Type 1 diabetes is a lifelong autoimmune condition that calls for thoughtful, ongoing, and individualized care.
The Valley Metabolic Health Type 1 Diabetes Clinic provides specialist support for adults living with type 1 diabetes across Nova Scotia’s Western Zone. Built on collaboration, connection, and community, we help you get the most from modern diabetes technology — insulin pumps and continuous glucose monitors — while working alongside your family physician and the broader diabetes care team. Our goal is steadier glucose, fewer lows, and a plan that fits your life.
What We Offer
Comprehensive, technology-forward care tailored to each person living with type 1 diabetes.
Assessment for pump therapy, initiation and onboarding, and ongoing optimization — including automated insulin delivery (hybrid closed-loop) systems from Tandem, Medtronic, Omnipod, and Ypsomed. We help you choose the right device, navigate the Nova Scotia Insulin Pump Program, and fine-tune settings as your needs change.
Setup and interpretation of CGM data from Dexcom and FreeStyle Libre systems, alarm and target customization, and using time-in-range and glucose trends to guide insulin adjustments and reduce hypoglycemia — with support accessing the Pharmacare Sensor Program.
Practical support for carbohydrate counting, insulin dosing, sick-day management, exercise, and preventing and treating lows — building the confidence to manage day to day.
Structured support for young adults moving from pediatric to adult diabetes care, and for anyone transferring into the Western Zone — so nothing falls through the cracks during a major change.
Routine screening for diabetes-related complications and coordination with your family physician, endocrinology, and allied health — keeping your whole care team aligned.

Our Team
Type 1 diabetes is best managed by a team. At the clinic, you’re supported by clinicians who work together on a single, shared plan.
Internal medicine and obesity medicine. Dr. Mindrum oversees medical management, insulin and pump optimization, and complex decision-making for patients living with type 1 diabetes.
Registered nurse and diabetes educator — hands-on pump and sensor training, dose adjustment, and the day-to-day coaching that makes diabetes technology work in real life.
Registered dietitian and diabetes educator — carbohydrate counting, nutrition, and meal planning tailored to life with type 1 diabetes.
We work alongside your family physician or nurse practitioner and regional diabetes programs — specialist input that strengthens, rather than replaces, your usual care.
Getting Started
New patients are seen by referral from a family physician or nurse practitioner. Here’s how it works.
Let your family physician or nurse practitioner know you’d like specialist support for type 1 diabetes — for pump therapy, CGM, or help getting your glucose on track.
Your provider faxes a referral to 1-855-962-2375, including your recent A1c, current insulin regimen, device details, and any relevant labs or complication screening.
Once your referral is received, the clinic contacts you to arrange your first appointment and explains what to bring and how to prepare.
