We help you choose the right device, navigate the Nova Scotia Insulin Pump Program, and fine-tune settings as your needs change.

Setup and interpretation of CGM data from Dexcom and FreeStyle Libre systems, alarm and target customization, and using time-in-range and glucose trends to guide insulin adjustments and reduce hypoglycemia — with support accessing the Pharmacare Sensor Program.

A compact, rechargeable pump whose Control-IQ technology adjusts background insulin and delivers automatic correction boluses using Dexcom CGM readings.

Medtronic MiniMed 780G

An automated insulin delivery system (SmartGuard) that auto-adjusts background insulin and auto-corrects highs using Medtronic’s own continuous glucose sensor.

Omnipod 5

A tubeless, waterproof pod worn on the body, with automated insulin delivery controlled from a handheld or a compatible smartphone.

Ypsomed mylife YpsoPump

A small, discreet pump paired with the mylife Loop app for automated insulin delivery. Try the explorer simulator to see how it works.

Continuous glucose monitors (CGM)

Dexcom (G6 / G7)

Real-time continuous glucose monitoring with customizable high and low alerts; integrates with Tandem and other automated insulin delivery systems.

FreeStyle Libre 3 (Abbott)

A small, discreet sensor that streams real-time glucose readings to your phone, with optional high and low alarms.

Disability Tax Credit

Disability Tax Credit certification

Many people with type 1 diabetes qualify for the federal Disability Tax Credit. Dr. Mindrum can complete the medical portion of your application. A $65 fee applies.

Start your application in CRA My Account (the digital DTC application) and note the reference number it gives you for your medical practitioner.

Pay & submit your code

Pay the $65 fee below and enter your CRA reference number with your payment so we can match it to your file.

We complete it

Dr. Mindrum completes the medical portion of your Disability Tax Credit application.

How to get referred

New patients are seen by referral from a family physician or nurse practitioner. Here’s how it works.

Talk with your provider

Let your family physician or nurse practitioner know you’d like specialist support for type 1 diabetes — for pump therapy, CGM, or help getting your glucose on track.

Referral to the clinic

Your provider faxes a referral to 1-855-962-2375, including your recent A1c, current insulin regimen, device details, and any relevant labs or complication screening.

We’ll be in touch

Once your referral is received, the clinic contacts you to arrange your first appointment and explains what to bring and how to prepare.
